Enrollment Agreement — submit once a space has been offered and accepted

Enrollment Checklist — describes forms due upon enrollment, before a child’s first day

For Enrolled Families

Income Verification — submit at enrollment + annually in July

Peace Books — submit at enrollment + annually in September

Topical Applications Permission — submit annually in July

ACH/Automatic Tuition Withdrawals — submit only when you are changing the designated financial account for automatic tuition withdrawals.

CACFP Food Program Enrollment — TCS participates in a federal “Child & Adult Care Food Program” (CACFP) that reimburses us for a portion of expenditures on “breakfast” and “afternoon snack” (check these two specific boxes). Print and submit hard copy to the office at enrollment + annually in July

Special Dietary Needs Medical Statement — this required CACFP form is for families with children who have special dietary needs. Print and submit completed hard copy to the front office at enrollment and if dietary needs change

Immunization Record — provided by child’s pediatrician or see front office for Statement of Exemption to Immunization Law; submit at enrollment and in conjunction with updated immunizations. Email or fax to director@treehousechildren.org / 802-649-1350.

Medication Administration Permission — submit as needed with sign-off from child’s health care provider.

Nursery only (Robin’s Nest)

Child & Family Profile, ages 0-1 year — submit at enrollment

Child & Family Profile, ages 1-2 years — submit at enrollment

Infant Formula/Formula Waiver, ages 0-1 year — print and submit this required CACFP form at enrollment. Alternately, hard copies are available at front office.

What is Your Baby Eating?, ages 0-1 year — print and submit this required CACFP form at enrollment. Alternately, hard copies are available at front office.
